We are hiring compassionate & dedicated
LICENSED PRACTICAL NURSES (LPN'S)
to provide medical support to our clients.
Duties and Responsibilities
Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
Provide direct nursing care to patients in their homes under the supervision of an RN or physician.
Administer prescribed medications, treatments, and therapies accurately.
Monitor patient health, vital signs, and symptoms, and report changes promptly.
Educate patients and family members on proper home care techniques and health maintenance.
Maintain accurate, detailed, and timely nursing documentation.
Perform wound care, catheter care, and tracheostomy care as needed.
Support patient emotional well-being and provide compassionate end-of-life care when necessary.
Ensure compliance with all healthcare regulations, agency policies, and individualized care plans.
Assist with mobility and transfers, ensuring patient safety and fall prevention.
Conduct patient assessments during each visit and update care plans as necessary.Participate in training sessions and professional development to maintain nursing licensure and skills.
Requirements
Valid LPN license in NY (REQUIRED)
Reliable transportation.
