RN Case Manager

Location Detroit, MI (The city is home to iconic Motown Music Museum, Belle Isle Park, and many other attractions.)

Pay Rate $1,800.00 – $2,700.00 per week
Position Summary The RN Case Manager will conduct comprehensive assessments of patients and families, develop and coordinate patient‑centered care plans, and collaborate with the care team to ensure seamless transitions and optimal outcomes.
Key Responsibilities
Conduct a comprehensive assessment of patient and family/caregiver’s biomedical, psychological, social and functional needs to gauge the potential impact on recovery.
Develop personalized patient‑centered care plans aimed at optimizing the patient’s care experience.
Engage patients and their families as part of the care team through advocacy, ongoing communication, health education, identification of resources and service facilitation.
Utilize professional judgment, critical thinking, motivational interviewing and self‑management techniques to assist patients in overcoming barriers to goal achievement.
Provide counseling and interventions related to treatment decisions and end‑of‑life issues including Advanced Care Planning.
Provide coordination as necessary to ensure patients seamlessly and safely transition between care settings.
Advocate for appropriate delivery of services within the patient’s health plan benefit structure.
Collaborate with appropriate members of the patient’s treatment/care team to co‑manage patients with complex medical and social needs; facilitate inter‑dependent collaborative care conferences.
Continuously evaluate the patient’s response to the care/treatment plan and make modifications when necessary.
Plan and participate in process improvement activities designed to reduce risk, including data collection, analysis and follow‑up intervention activities.
Facilitate interventions in cases involving child abuse and neglect, domestic violence, elder abuse, institutional abuse and sexual assault.
Support department‑based goals that contribute to the success of the organization.
Perform other duties as assigned.
Minimum Required Qualifications
Unrestricted Michigan RN license
BLS certification through AHA
5 years of recent inpatient case management experience (telephonic, utilization management, or therapy experience excluded)
Acute discharge planning experience
Epic EMR experience
Strong computer skills
Assignment Details Length of Assignment: 13 weeks Shift / Hours Per Week: Days 8a‑4:30p, rotating weekends
