Strategic Staffing Solutions (S3) is hiring an RN Inpatient Case Manager
Location:
Detroit, MI (Onsite)

W2 contract, 3+ months

Mon - Fri, 8am - 5pm
Pay Rate:
$50-55/hr
Role Summary The Integrated Case Manager for Population Health is an interdependent member of the patient‑centered care team. Responsibilities include assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet comprehensive health care needs and promote patient safety, quality of care, and cost‑effective outcomes. This role serves patients who have experienced a critical event or diagnosis that requires complex management strategies across ambulatory, inpatient, or health plan settings.
Responsibilities
Conduct comprehensive assessments of patients and families to gauge potential impact on recovery.
Develop personalized, patient‑centered care plans to optimize the care experience.
Engage patients and families through advocacy, ongoing communication, health education, and resource facilitation.
Utilize professional judgment, critical thinking, motivational interviewing, and self‑management techniques to help patients overcome barriers to goal achievement.
Provide counseling regarding treatment decisions and end‑of‑life issues, including advanced care planning.
Coordinate transitions to ensure patients move safely between care settings.
Advocate for appropriate delivery of services within the patient’s health plan benefit structure.
Collaborate with the treatment team to co‑manage complex medical and social needs and facilitate inter‑care conferences.
Continuously evaluate patient response to care plans, making adjustments as needed.
Participate in process improvement activities to reduce risk, including data collection, analysis, and intervention follow‑up.
Facilitate interventions related to child abuse, domestic violence, elder abuse, institutional abuse, and sexual assault.
Support department goals that contribute to organizational success.
Required Skills
Minimum 5 years of acute inpatient case management experience (excludes telephonic, utilization management, or therapy roles).
Proficient in Epic EMR.
Excellent verbal communication and written documentation skills.
Strong customer service and interpersonal skills with ability to interact across all organizational levels.
Strong problem‑solving, analytical, and decision‑making skills.
Proficient computer skills and knowledge.
Preferred experience in discharge planning, home health care, rehabilitative medicine, community health, or managed care.
Knowledge of preventive service guidelines, clinical practice guidelines, behavior change theory, Medicare/Medicaid regulations, and case management principles.
Knowledge of medical ethics and legal implications related to case management.
Understanding of social determinants of health and their impact on patient well‑being.
Comfortable facilitating community resources for diverse populations.
Strong organizational, planning, and implementation skills to manage multiple complex patient needs.
Compassionate advocacy for patients and families.
Education
Nursing degree or MSW.
Certification & Licensure
Registered Nurse (RN) with a valid, unrestricted Michigan license OR Licensed Social Worker (LMSW) with a valid, unrestricted Michigan license.
