Full-time TASC Care Manager
to join our team in
Charlotte, NC . How You’ll Make an Impact As a
TASC Care Manager , you will play a key role in helping clients involved in the TASC (Treatment Accountability for Safer Communities) program access the support and services they need to achieve stability and recovery. You’ll work closely with the criminal justice system, treatment providers, and clients to ensure accountability, progress, and success. You will: Screen and assess clients referred to by probation officers and other community partners. Provide individual case management, including care coordination, court reporting, communication with probation officers, urinalysis monitoring, fee collection, and monthly progress reports. Conduct comprehensive assessments to determine treatment needs and eligibility. Develop and implement individualized treatment plans that align with client goals and program requirements. Serve as a liaison between the criminal justice system and treatment providers, ensuring clear communication and coordinated care. Monitor compliance with treatment plans and court orders through regular check-ins and progress updates. Document all client interactions accurately and timely in electronic health records. Provide crisis intervention and support to help clients maintain stability and safety. What We’re Looking For Education:
Bachelor’s degree in
criminal justice or behavioral science
required. Experience:
Previous experience in case management, substance use treatment, or criminal justice preferred. Skills:
Strong problem-solving and interpersonal communication skills. Excellent written and verbal communication. Proficient in Microsoft Office (Outlook, SharePoint, OneDrive, Teams, etc.).
Licensure/Certification:
