Job Summary
Located on Nantucket island. The Palliative Care Nurse Specialist is responsible for providing direct patient care and clinical management for patients and families. The Palliative Care Nurse works collaboratively with the Nurse Practitioner Director of PASCON to provide comprehensive palliative care for patients and families. The Palliative Care Nurse Specialist collaborates with physicians, Mid-level providers, nurses, social workers, and other appropriate staff within the hospital organization, as well as with outside agency staff to ensure team-based, coordinated, comprehensive care and seamless transitions for patients in any setting (hospital, home, nursing home). Essential Functions
Works collaboratively with Palliative Care Department Nurse Practitioner Manager to Organize, develop, plan, implement, and evaluate Palliative & Supportive Care Program goals, objectives, policies, and procedures necessary for a comprehensive program, in accordance with hospital policy and appropriate standards.
Provides high quality direct patient care, including education and counseling, to departmental patients in any setting – home, hospital, office, or nursing home.
Performs and records the patient history and physical exam and discusses findings with the NP Manager and physician as needed.
Follows and manages patients (in collaboration with the NP manager and physician) enhancing continuity of care.
Performs, oversees, and assures completion of specialized procedures mutually agreed upon with collaborating physician.
Drivers License and vehicle for visits required.
Qualifications Education
Bachelor\'s Degree Nursing Required And Master\'s Degree Nursing Preferred
Can this role accept experience in lieu of a degree? No
Licenses and Credentials
MA Registered Nurse License required
Valid Drivers License and vehicle for visits required.
Experience
2-3 years of nursing experience required
2-3 years of hospice and palliative nursing preferred

Effective collaboration skill ability with all levels.
Ability to rapidly make accurate decisions, analyze, and interpret information.
Strong communication and interpersonal skills with demonstrated ability to teach and develop others.
