We are seeking a visionary and dynamic provider who is creative in problem solving and contributes new ideas to join our team in Integrated Clinical Services. Responsibilities

Diagnose, manage and treat acute and chronic medication conditions including prenatal care, according to the scope of practice; perform medical history and physical exams, order and interpret diagnostic tests, devise treatment plans, discuss risks/benefits, and prescribe and monitor medications. Utilize panel-based information technology tools to prioritize and direct proactive care for patients in the provider panel, including preventive health screening and chronic disease management. Provide follow-ups on utilization issues (ED, hospital admissions). Establish need for specialty and rehabilitative services and make appropriate referrals. Assess and counsel patients regarding preventive care needs; deliver services or referrals as appropriate. Determine need for high-risk referrals and coordinate scheduling. Seek clinical consultation from other providers as needed and provide consultation to nurses and support staff. Prioritize and direct activities of the care team; coordinate with Behavioral Health providers to integrate care. Provide coverage for clinical colleagues in their absence. Complete patient documentation and manage electronic in-basket per policies and procedures. Participate in quality improvement and assurance projects as needed. Qualifications

Minimum Qualifications / Transferable Skills

Master's Degree in Nursing (NP) or PA and 3 to 6 years of experience in related duties. License to practice medicine as a Nurse Practitioner or Physician Assistant as per Oregon Revised Statutes (ORS) 678.375. CPR certification. A combination of experience and training that provides the knowledge and abilities needed; non-traditional education/experience evaluated. Education/Training

Master’s Degree in Nursing (NP) or Physician Assistant program. Preferred Qualifications / Transferable Skills
