Position Summary
Responsible for assisting in implementing CrescentCare’s community outreach and HIV/HCV testing services as outlined in the Metropolitan Human Services District (MHSD) grant. This role targets high-risk substance users, especially pregnant individuals, women with dependent children, and people who inject drugs (PWID). Duties include conducting street-based outreach, HIV/HCV/STI testing and counseling, linkage to medical and substance use services, and coordination of MHSD-specific documentation and reporting deliverables. Prevention Coordinator II plays a key role in ensuring CrescentCare’s outreach efforts meet quality standards and grant requirements. Work with linkage and other programs to ensure clients access all the services needed. Ensure compliance with funder requirements and attainment of program deliverables.
Provide client-centered HIV/STI/HCV counseling, testing, outreach, and referrals to high-risk individuals in field and clinical settings
Follow the implementation of a scientifically sound, evidence-informed outreach model (e.g., NIDA model) to engage substance users in harm reduction and treatment
Conduct individual risk assessments and develop personalized risk-reduction plans
Maintain confidentiality and supportive counseling during pre-/post-test sessions
Coordinate referrals to OPH and other services for clients who test positive or require additional care
Track services using MHSD documentation tools, including monthly spreadsheets, client logs, and follow-up data
Work in collaboration with MHSD team lead, Prevention outreach team programs, internal linkage teams, substance use providers, and maternal health navigators to ensure access to wraparound services
Distribute safer sex materials, syringes (if applicable), and hygiene products as part of harm reduction efforts
Coordinate program activities to achieve deliverables of MHSD grant
Actively participate in community events and mobile outreach operations including set-up and breakdown
Assist with data entry, reporting, and documentation required by MHSD
Attend MHSD trainings, annual meetings, and district public forums as required
Represent CrescentCare and MHSD values in community engagement activities

Qualifications Education
Bachelor's degree and 1 year of experience in public health, harm reduction, or social services; or
High School Diploma/GED and 3 years of relevant experience
Other (licensure, Certifications)
Experience with substance-using populations, women’s health, HIV/HCV services, or peer navigation
Valid Louisiana Driver’s License required
Willingness to work flexible hours, including evenings and weekends
Prior HIV/HCV testing and counseling certification (or ability to complete training within 60 days)
Bilingual Spanish a plus
Essential Physical Requirements
Extended periods of sitting required.
Work is primarily office based.
Some travel may be required.
May need to lift up to 25 pounds in equipment and supplies
