POSITION SUMMARY:

This position acts as a coordinator between internal and external referral sources, and acute inpatient rehab operations. The Clinical Rehabilitation Specialist is responsible for establishing relationships with case managers, physicians, payors and other referral sources. The Clinical Rehabilitation Specialist will ensure appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Management and the Rehab Medical Director. The Clinical Rehabilitation Specialist works closely with hospital case management to ensure a responsive and smooth flowing referral and admission process for the inpatient acute rehab program. Assist and coordinate the in-take and pre-admission screening process. Accurately documents evidence of rehab admission criteria on Pre-Screen evaluation in a timely and legible manner. Performs on-site and off-site clinical evaluation to assist Rehab Medical Director in determining appropriateness of patient for admission. Demonstrates clinical knowledge of rehab admission criteria, leading to appropriate identification of qualified rehab patients. Advises referral sources on inpatient rehab criteria. Ensures referral sources receive appropriate and timely follow-up communications regarding their patients and hospital programs. Functions as primary contact for case management and referring physicians. Establishes and maintains effective working relationships with the same referral sources. Proficient in time management. Prioritizes meeting commitments to referral sources. Develops a call-plan to prioritize utilization of time. Completes face-to-face, in-service educational contacts with physicians, case managers, other interdisciplinary members and other professionals. Serves as an educational resource to patients, family members and caregivers. Responsibilities

Coordinate intake and pre-admission screening and document evidence of admission criteria. Conduct clinical evaluations to support admission decisions with the Rehab Medical Director. Identify appropriate rehab candidates and communicate criteria to referral sources. Serve as primary liaison with case management and referring physicians; maintain relationships with referral sources. Manage time effectively and develop plans to optimize outreach and utilization of time. Provide educational contacts and resources to patients, families, and staff. Requirements

Graduate of an accredited school in clinical specialty Minimum 2 years of recent clinical care experience Licensed Professional in a clinical health care specialty (e.g., RN, Physical Therapist, Occupational Therapist, or Speech Therapist) Pass Functional Independence Measurement (FIM) within 90 days upon hire, then maintain current certification If RN, CRRN preferred or achieved within 2 years of starting BLS certification (AHA) required Clinical experience in inpatient rehabilitation care and management; experience as a clinical liaison coordinating admissions Critical thinking, service excellence, interpersonal skills, ability to read and follow instructions, PC skills What qualifications you will need

Bachelor’s Degree (RN) Registered Nurse, or (OT) Occupational Therapist, or (LPT) Licensed Physical Therapist, or (SLP) Speech-Language Pathologist Benefits
