Speech Therapist – Pediatric – Fee‑For‑Service – Home Health
Your Choice At Home is an affiliate of Cobble Hill Life Center and a Certified Home Health Agency (CHHA). We provide reliable, dignified care 24/7 in the patient’s home, specializing in speech/language rehabilitation and swallowing services.
Responsibilities
Develop and implement individualized treatment plans, including speech, language, and swallowing modalities; determine frequency of visits and short‑term goals.
Assess patients, organize therapeutic care, and evaluate test results to determine the presence and extent of speech and language disorders or swallowing disorders.
Collaborate with agency professionals to evaluate rehabilitation potential and recommend therapeutic devices/equipment to maintain or restore maximum performance of speech and language.
Provide speech and language rehabilitation services to achieve identified outcomes.
Supervise and direct other health team personnel in the use of verbal/physical exercises and equipment to facilitate speech and language.
Teach patients, families, and health team personnel the use of adaptive equipment and continuation of maintenance programs.
Counsel patients/families to manage functional limitations and adjust to the patient’s functional limitations.
Observe, record, and report patient response to treatment and any changes to the physician and program personnel.
Qualifications
Valid license and current registration as a Speech Pathologist in New York State.
Master of Science in Speech‑Language Pathology.
Minimum 1 year experience as a speech pathologist in hospital, long‑term care, rehab facility, or home health agency.
Experience working with pediatric population is a plus.
Comfortable making home visits in potentially unsafe/unsanitary environments and traveling on public transportation in unsafe areas.
Ability to lift up to 50 pounds unassisted and up to 325 pounds using an assistive device in the home environment.

Salary: $85.00 per visit
