This role provides pediatric speech and language rehabilitation services to patients in their homes. The speech therapist plans, delivers, and evaluates treatment, collaborates with families and agency professionals, and uses assistive devices as needed.
Responsibilities
Develops a plan of treatment that includes modalities, visit frequency, outcomes, and short‑term goals.
Assesses patients and develops a therapeutic plan of care for speech, language, and swallowing disorders.
Evaluates test results to determine the presence and extent of speech, language, or swallowing disorders.
Collaborates with agency professionals to evaluate patient limitations and rehabilitation potential.
Recommends therapeutic devices and equipment to maintain or restore maximum performance of speech and language.
Considers the patient’s psychological response and changes in body image related to disease.
Addresses patient needs in a realistic short‑term and long‑term treatment plan.
Implements the plan of treatment under physician direction and in collaboration with agency professionals.
Provides speech and language rehabilitation services to achieve identified goals.
Plans, directs, and supervises family and other health team personnel in using specific verbal/physical exercises and equipment to facilitate speech and language.
Teaches the use of adaptive equipment to patients, family, and other health team personnel.
Instructs patients, family, and other health team personnel in program exercises for continuation of maintenance program.
Counsels patient/family to aid in interaction and adjustment to patient functional limitations.
Observes, records, and reports to the physician and program personnel the patient’s reaction to treatment and any changes.
Required Qualifications
Licensure and current registration as a Speech Pathologist in the State of New York.
Master’s of Arts in Speech Pathology.
Minimum of one year experience as a speech pathologist in a hospital, long‑term care, rehab facility, or home health agency.
Experience working with pediatric populations is a plus.
Ability to make home visits in potentially unsafe and unsanitary home environments and be comfortable with travel on public transportation in potentially unsafe areas of the community.
Lifting up to 50 pounds unassisted and up to 325 pounds utilizing an assistive device in the home environment may also be required.
