Job Overview You will play a pivotal role in delivering and coordinating comprehensive multidisciplinary home health services to patients within the community. You will collaborate closely with physicians and team members, implement tailored care plans, and ensure services are cost‑efficient and aligned with insurance requirements. Key responsibilities include assessing patient needs, completing initial evaluations, delivering direct care, accurately completing OASIS assessments, managing diagnosis coding, and participating in weekly PPS meetings.
Responsibilities
Assess patient needs and complete initial evaluations.
Deliver direct care in patients’ homes or other facilities.
Accurately complete OASIS assessments and manage diagnosis coding.
Participate in weekly PPS meetings and report on service adherence.
Educate patients and families on disease management and self‑care strategies.
Coordinate care and maintain accurate record‑keeping using relevant software tools.
Travel efficiently within Brooklyn, Queens, and Manhattan as required.
Qualifications
Minimum one year of nursing experience.
Preference for background in long‑term care or Certified Home Health Agency (CHHA) settings.
Critical thinking and problem‑solving skills essential for navigating varied home environments.
Reliable transportation and ability to travel within Brooklyn, Queens, and Manhattan.
Physical stamina to stand, walk, bend, and lift up to 50 pounds unassisted or 325 pounds with assistive devices.
Proficiency in documentation and communication software tools.
Salary $85.00 - $105.00 per visit
