Overview We are seeking a qualified

Senior Human Services Care Counselor (SHSCC)

to fill a Limited Term position within the Transitional Housing Program Plus (THP+). The SHSCC will serve as the primary contact for THP+ program participants, provide on‑site residential support, coordinate independence workshops, ensure rent and subsidy entitlements, and track graduation and trust fund entitlements.

Base Pay Range $101,441.60



$126,817.60

per year

Responsibilities

Provide case management support to THP+ program participants housed at multiple sites.

Serve as the primary point of contact for THP+ participants and coordinate workshops supporting resident independence.

Ensure monthly rent and living subsidy distribution and track graduation and trust fund entitlements.

Work a 24/7 schedule, including evenings, weekends, holidays, and after‑hours on‑call coverage.

Perform duties associated with the administration of the residential facility, including personnel transactions, program planning, budget preparation, shift scheduling, and preparation of periodic and special reports.

Assist supervisors in supervising daily operations, coordinating residential services, training program personnel, and evaluating performance.

Assist in preparing related periodic and special reports on facility operation.

Assist in preparing and supervising volunteers and explaining the program to public and community groups.

Perform related duties as assigned.

Qualifications

Two years of experience working with youth in a residential setting.

Three years of experience providing case management services, including identifying clients’ needs, developing individual case plans, and monitoring progress.

Strong interpersonal and relationship‑building skills to work effectively with community‑based organizations and stakeholders.

Excellent organizational and written/oral communication skills.

Flexibility and the ability to manage time and multiple tasks.

Able to work independently and with minimal supervision.
