Job Summary We are seeking a full‑time, 40‑hour LPN Lead to support this office on‑site. Our practice is open Monday through Friday from 8:00am to 5:00pm.

Essential Functions

Provide direct patient care, including administering medications, performing treatments, and assisting with activities of daily living.

Provide leadership and guidance to a team of LPNs and nursing support staff.

Collaborate with RNs, physicians, and other healthcare providers in the development and implementation of individualized care plans.

Educate patients and their families about their conditions, treatment plans, and self‑care techniques.

Administer medications and ensure accurate documentation of medication administration.

Maintain accurate and comprehensive patient records, including assessments, care plans, progress notes, and medication administration records.

Qualifications

Education: Trade/Technical/Vocational Diploma, Licensed Practical Nurse required.

Licenses and Credentials: Licensed Practical Nurse (LPN – State License) required. Basic Life Support (BLS Certification) preferred.

Nursing Experience: 2–3 years required.
