Care Continuity

About Care Continuity Care Continuity is redefining patient navigation. We combine clinical expertise, AI‑driven insights, and compassionate human support to ensure patients receive the care they need—when and where they need it. Our solutions empower health systems and providers to close care gaps, reduce readmissions, and drive ROI through smarter, more connected navigation. Our work is transforming how care decisions are made—and we’re just getting started.

About The Role As a Team Lead for our Integrations team, you will lead the design and delivery of integrations with health system partners and external platforms. This is a hands‑on role: you’ll build integrations yourself while also setting technical standards, mentoring engineers, and shaping the future of our integration function.

What You’ll Do

Own the design, development, and implementation of integrations with external systems (APIs, HL7, FHIR, CDA, flat files, unstructured clinical documents)

Lead a small team of integration engineers, providing technical guidance and mentorship

Partner with client‑facing teams to enable smooth deployment and integration into customer environments

Collaborate with QA and product teams to ensure integrations are reliable, scalable, and well‑tested

Diagnose and resolve integration issues, optimizing performance, scalability, and timeliness of data delivery

Monitor key performance metrics (e.g., latency, throughput) and lead continuous improvement efforts

Stay current with new interoperability standards and technologies, evaluating potential adoption

Define and maintain integration best practices, coding standards, and documentation processes

What We’re Looking For

5+ years of experience in integration engineering, interoperability, or healthcare IT

Demonstrated ability to mentor, guide, or manage other engineers

Strong experience with C# / .NET in a Microsoft Azure cloud environment

Familiarity with databases (SQL and NoSQL, e.g., MongoDB, Redis)

Experience with CI/CD pipelines and infrastructure‑as‑code (Azure DevOps, Terraform)

Understanding of HL7, FHIR, CDA, flat file formats, and API‑based integrations

Strong problem‑solving skills and ability to collaborate across teams

Experience working in regulated environments (HIPAA, SOC‑II, HITRUST) is a plus

Compensation & Benefits

Estimated salary range: $115,000 – $160,000 per year, depending on skills and experience

Comprehensive benefits package, including medical, dental, vision, and 401(k)

Equity opportunities

Flexible PTO and fully remote work environment

Salary: $115,000 - $160,000 per year
