Care Continuity
About Care Continuity
Care Continuity is redefining patient navigation. We combine clinical expertise, AI-driven insights, and compassionate human support to ensure patients receive the care they need - when and where they need it. Our solutions empower health systems and providers to close care gaps, reduce readmissions, and drive ROI through smarter, more connected navigation.
Our work is transforming how care decisions are made - and we're just getting started.
About The Role As Lead Integration Engineer at Care Continuity, you'll design, build, and maintain the data pipelines and system integrations that connect our platform with customer systems and third-party endpoints. You'll play a key role in ensuring interoperability across diverse healthcare data sources while mentoring teammates and shaping integration best practices.
This is a hands‑on engineering role with opportunities for leadership and cross‑team collaboration.
What You'll Do
Design, develop, and implement integrations with external systems (APIs, HL7, FHIR, CDA, flat files, unstructured clinical documents)
Partner with client‑facing teams to enable smooth deployment and integration into customer environments
Collaborate with QA and product teams to ensure integrations are reliable, scalable, and well‑tested
Diagnose and resolve integration issues, optimizing performance, scalability, and timeliness of data delivery
Monitor key performance metrics (e.g., latency, throughput) and lead continuous improvement efforts
Mentor engineers on integration practices, fostering knowledge‑sharing and technical growth
Stay current with new interoperability standards and technologies, evaluating potential adoption
Document integration designs, processes, and lessons learned to build organizational knowledge
What We're Looking For
5+ years of experience in integration engineering, interoperability, or healthcare IT
Strong experience with C# / .NET in a Microsoft Azure cloud environment
Familiarity with databases (SQL and NoSQL, e.g., MongoDB, Redis)
Experience with CI/CD pipelines and infrastructure‑as‑code (Azure DevOps, Terraform)
Understanding of HL7, FHIR, CDA, flat file formats, and API‑based integrations
Strong problem‑solving skills and ability to collaborate across teams
Experience working in regulated environments (HIPAA, SOC‑II, HITRUST) is a plus
Compensation & Benefits
Estimated salary range: $115,000 - $160,000, depending on skills and experience
Comprehensive benefits package, including medical, dental, vision, and 401(k)
