You can find us in schools, homes, the community, and our Taunton offices empowering our clients with the latest evidence-based behavioral health treatments. We are a non-profit community mental health center serving 50 communities in Massachusetts and growing! CCBC provides inpatient, outpatient, and urgent care services to adults and children overcoming depression, anxiety, mood disorders, substance abuse, and many other conditions hindering personal fulfillment. Be a part of this innovative program that is the first of its kind in Massachusetts. CCBC and the Department of Mental Health (DMH) will work collaboratively in the PACT- Y program to meet the needs of youth and families who want a creative, flexible, dynamic team to support them in achieving their goals. As a Mentor/Case Manager you will work within a multi-disciplinary team to improve the social, emotional and recreational functioning of youth as they transition to adulthood. The Mentor/Case Manager will provide opportunities in the community for youth to practice skills, assist with obtaining jobs, explore after-school activities and support goals identified on the team treatment plan. Responsibilities
Provide case management services to increase skills related to independent living, community integration, employment, recreation and housing Assist families in obtaining resources to improve family functioning within their community Support and teach skills related to the treatment plan Model skills and techniques in the community to increase youth’s feelings of competence and self-esteem Ensure the youth has a voice in treatment planning and identification of goals Complete required documentation per program guidelines Coordinate care with the family, team, providers and DMH Provide on-call and weekend support on a rotating basis Facilitate change based on youth and family needs, strengths and preferences Qualifications And Education Requirements
Bachelor’s degree in psychology, social work or a related field Some prior experience working with children, youth and families with mental health challenges A valid driver’s license and reliable transportation Willingness to learn and seek guidance as needed Good organizational skills, in addition to effective verbal, written and computer skills Community Counseling of Bristol County is an equal opportunity employer and welcomes applications from diverse candidates.
