Sun River Health is currently seeking part-time Psychiatric Nurse Practitioner to join our team supporting the Southampton Health Center site located in Southampton, NY. A Psychiatric Nurse Practitioner provides psychiatric clinical services for patients. Performs job duties in compliance with systems and job specific standards. Responsibilities
Consult with MD/Psychiatrist/LCSW on patients via eCW, telephone encounter, and/or with patient visits Consult with MD/Psychiatrist on medication management, via eCW, telephone encounter, and/or visit with patient as needed to address BH concerns Provide clinical care (medication management/psych evaluations) for patients to support efforts to achieve care plan goals, in support of ongoing care by MD/Psychiatrist/LCSW, with referral back to ongoing care with clinical team Participate in case conferencing and high-risk meetings with LCSW/MD/Psychiatrist and care teams Approval of treatment plans for patients in BH care at Sun River Participate in efforts to support Sun River’s primary care and BH integration efforts Ensure appropriate transitions of care with external providers, PCP, hospitals, etc. Respond to Sun River clinical teams for crisis intervention Interviews, diagnoses and evaluates patients referred for psychiatric assessment Participates as a team member in conferences concerned with diagnosis and treatment planning Writes reports and documentation as required Consults with other mental health and medical provider staff regarding treatment of patients Prescribes psychotropic medication as needed and monitors its effectiveness Provides in-service training to professional and paraprofessional staff Reviews case records to monitor quality of care Must demonstrate knowledge and skill necessary to provide care appropriate to the age of the patients served Must demonstrate knowledge of the principles of growth and development over the life span Must be able to identify and categorize each patient’s age–specific grouping of needs, such as those for infant, adolescent, or geriatric patients Must be familiar with the process of diagnosing and treating patients with substance use disorder and make appropriate referrals when necessary Minimum Education & Licensure
Minimum Education Requirement: Master’s Degree Minimum Licensure: Nurse Practitioner in Psychiatry and CPR/BLS Required Skills & Knowledge
Knowledge of diagnostic and treatment procedures that nurse practitioners are trained to do and are necessary for practicing in health care centers. Competence in delivery of health care to patients (including preventative health education) Ability to collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.
