Job Summary This Patient Navigator position will support an antenatal patient navigation research program,

Partnering with Antenatal Navigators to Transform Health in Pregnancy (PATH) . The study aims to improve maternal health outcomes among low-income pregnant individuals by implementing a barrier-focused, long-term, patient-centered intervention that offers support for specific health services. In this NIH-funded study, pregnant individuals will be randomized to either receive intensive patient navigation throughout pregnancy until two weeks postpartum, or receive usual antenatal care.

Responsibilities

Lead execution and control of biomedical and social science research projects.

Provide patient navigation services, including counseling and education to help patients effectively use healthcare services.

Explain study details to patients and provide contact information.

Coordinate support services considering social determinants of health.

Identify social/structural needs and offer appropriate referrals.

Serve as a bridge to clinical providers, offer limited health education, and psychosocial support.

Be extensively trained in community resources, prenatal needs, study protocols, and barriers to care.

Develop individualized pregnancy care plans and schedule appointments.

Offer social, emotional, and logistical support.

Perform health education on contraception and breastfeeding.

Support administrative tasks like maternity leave paperwork and insurance needs.

Document patient information and navigation services accurately in databases.

Coordinate data processing, analysis, and research activities.

Collaborate with clinicians and healthcare team regarding patient enrollment and care.

Maintain organized records and develop research methodologies.

Contribute to developing a women’s health patient navigation model and maintain documentation manuals.

Ensure completion of study activities, including recruitment and obtaining informed consent.

Interview patients and obtain medical and social histories.

Administrative and Other Responsibilities

Manage data collection, maintenance, analysis, and evaluation for grant submissions, presentations, and publications.

Ensure accurate data entry and generate reports on navigation activities.

Assist in reviewing, analyzing, and preparing research documents and reports.

Coordinate with sponsoring and collaborating organizations.

Connect patients to community programs and support services.

Conduct outreach and build relationships with healthcare and social service organizations.

Ensure compliance with regulatory guidelines and research protocols.

Order and maintain study supplies and participant compensation records.

Participate in staff development and training activities.

Minimum Qualifications

Bachelor’s degree in social or health sciences or related field, or equivalent experience with 3 years’ relevant experience; or Master’s degree with 1 year relevant experience.

Supervisory or project management experience.

Completion of NU’s IRB CITI training before interacting with participants, re-certifying every 3 years.

Ability to work independently and as part of a team.

Interest in women’s health and patient advocacy.

Preferred Qualifications

1 year experience as a patient navigator or in clinical settings.

Bilingual in English and Spanish.

Experience in women’s health and outpatient support.

Preferred Skills

Comfort with electronic communication methods.

Experience in health education and developing educational materials.

Additional Information Target salary range: $51,346 - $56,000, based on experience and qualifications.
