Responsibilities

Provide outpatient rehabilitation services in an in‑home setting for the older adult population.

Evaluate and treat in the patient’s home, assisted living facility, or independent living facility.

Conduct screenings, history, systems review, and appropriate tests to establish diagnosis and plan of care.

Develop and implement treatment goals and methods in collaboration with patients and caregivers.

Apply therapeutic techniques in areas such as cognitive‑communication, speech intelligibility, oral motor skills, swallowing disorders, perceptual abilities, orientation, memory, pragmatics, psychosocial expression, functional communication, and case management skills.

Coordinate acquisition of adaptive communication devices and instruct patients in their use.

Supervise CFY temporary licensees, students, and support personnel per state licensure requirements.

Maintain timely clinical and billing documentation and attend staffing conferences and related meetings.

Make referrals to other disciplines and consult with the interdisciplinary team and physician.

Promote a safe environment and enforce elimination of fire and safety hazards.

Perform routine reexamination and modify plans of treatment as needed.

Perform other related duties as required.

Qualifications

Valid Speech‑Language Pathologist license in the state(s) of practice, or proof of license eligibility.

Certificate of Clinical Competence (ASHA) or eligibility to participate as an ASHA Clinical Fellow.

Valid driver’s license or reliable, independent transportation to and from patients’ residences.

Posted Salary Range USD $48.00 – USD $62.00 /Hr.
