Overview
Clinical Pharmacist – Site-Based Oversight (Novant Health) delivers optimal medication management across all dimensions of care. This role partners with providers and clinical staff to ensure high-quality, evidence-based, and cost-effective pharmaceutical care. This position involves scheduled monthly site visits to affiliated practices, focusing on medication safety, documentation, and communication with Practice Managers (PMs). Key Responsibilities
Site Visit Oversight: Coordinate monthly visits with Practice Managers (PMs), scheduling during regular business hours based on mutual availability. Time commitment: Spend approximately one hour per practice per month, including travel, on-site evaluation (30–60 minutes), and follow-up documentation. Conduct medication safety checks using a standardized checklist. Submit completed documentation to PMs and assist in addressing any identified corrections or follow-ups. Medication Management: Ensure patients receive the correct medications at appropriate dosages and timing. Support providers with medication therapy management strategies. Patient Counseling: Educate patients about proper medication use, side effects, and interactions. Interdisciplinary Collaboration: Work closely with physicians, nurses, and other healthcare professionals to enhance care delivery and medication safety. Quality Improvement: Participate in initiatives to monitor, evaluate, and improve medication-related processes. Education & Precepting: Serve as a mentor to pharmacy students and residents, supporting experiential learning. Qualifications
Education: Doctor of Pharmacy (PharmD) required. Experience: Completion of a PGY1 residency or two years of experience in a health system preferred. Licensure: Active pharmacist license required. Preferred: Board certification in Ambulatory Care (BCACP) or Pharmacotherapy (BCPS). Additional Information
