St. Paul’s PACE Program
San Diego’s St. Paul’s PACE (Program of All-Inclusive Care for the Elderly) offers coordinated medical, social and home care services for individuals 55 years or older who have chronic illness or disabilities and need ongoing support to live independently in their home and community.

Job Summary Under the direction and supervision of the RN Clinic Supervisor, the RN will conduct comprehensive nursing assessments—initial, semi‑annual, and annual—and triage participants as needed. The RN plans and implements interventions according to individualized care plans, evaluates outcomes, administers medications and primary care orders, and documents participant complaints. The RN provides education to participants and caregivers, and communicates participants’ conditions to the multidisciplinary team.
Qualifications and Requirements
Graduate of a professional nursing program; current California RN license and CPR certification required.
Minimum one year experience with the frail or elderly population preferred; IV certification preferred; related training or experience acceptable.
Bilingual preferred.
Physical Demands and Working Conditions Good mental and physical health required. Tasks include standing, walking, lifting patients, manual dexterity, and repetitive use of hand and finger movements. Must be able to transfer participants who need maximum assist of one, lift/carry up to 50 pounds, and push or pull objects up to 150 pounds, using proper body mechanics.
