San Diego’s St. Paul’s PACE program provides coordinated medical, social, and home care for individuals 55 years or older with chronic illness or disabilities, enabling independent living in home and community.
Signing Bonus: $1,500 for PACE LVNs (conditions apply).
Schedule:
Wednesday – Sunday 8:00 AM – 4:30 PM
Site:
Home visits throughout San Diego County; reliable transportation required.
Job Summary:
Under the direction of the In‑Home Services Director, provides skilled nursing services (medication administration, wound care, etc.) to participants in their homes. Maintains thorough documentation and communicates participant changes and progress to appropriate staff.
Qualifications and Requirements
Education
– Licensed Vocational Nursing graduate; current California L.V.N. license, CPR and first aid certification required; at least 18 years of age.
Experience
– Minimum one year’s experience working with frail or elderly population; relevant training or related job experience preferred.
Bilingual preferred (Spanish).
