Under the clinical direction and supervision of the Medical Director, provide primary care to St. Pauls PACE program participants. Perform physical assessments, develop and contribute to plans of care, evaluate physical complaints, provide appropriate treatment, and deliver participant and caregiver education. Participate as a member of the Interdisciplinary Team and apply knowledge to assess, plan, care for, and provide services to frail elder participants according to PACE standards. Responsibilities
Provide primary care and management of participants under clinical direction of the Medical Director. Conduct physical assessments and develop care plans for PACE participants. Treat acute and chronic conditions and educate participants and caregivers. Collaborate with the Interdisciplinary Team to coordinate medical and home care services. Adhere to PACE standards and deliver care to frail elder participants. Qualifications and Requirements
Education: Graduate of an accredited Physician Assistant or Nurse Practitioner program. Licensure: Licensed as a Physician Assistant or Nurse Practitioner in the State of California. Certification: Certified as a Physician Assistant or Nurse Practitioner by a nationally recognized certifying organization (as applicable in CA). Experience: Minimum of one year working with frail and elderly populations; two years in geriatric settings is desirable. Training or related experience relevant to the role is required. Required Credentials
Valid California license as a Physician Assistant or Nurse Practitioner Degree from a NCCPA-certified program DEA Number Skills and Knowledge
Knowledge of home and community-based care for the elderly and disabled adults. Understanding of PACE regulations (preferred). Knowledge of physical, mental, and social needs of frail older adults. Ability to lead in an interdisciplinary setting and strong organizational skills. Ability to adapt to changing priorities in the PACE program and agency. Experience with frail or elderly populations.
