Overview Salary range for this position is $70,000 - $75,000 annualized based on skills and experience. Victory Housing offers a competitive benefits package including medical, dental, vision, and life insurance as well as retirement and paid time off.

Job Summary Implementation, coordination and supervision of all aspects of a comprehensive Health, Wellness and Clinical Program which addresses the needs of all Assisted Living Residents in accordance with Victory Housing, Inc. Policies and Procedures, COMAR 10.07.14 and Board of Nursing Regulations and Standards.

Oversight Includes Provide nursing assessments, care planning, care oversight and quality assurance for all residents. Maintain all charts to include progress notes, identification of nursing interventions, treatments and assessments. Implement a Wellness Program to include weekly scheduled resident Health and Wellness Check-ups, to assure a proactive and preventative clinical approach. Oversight and coordination of third party vendors (e.g., podiatrists, dentists, labs, pharmacy, home health, hospice, etc.). Fulfill the role of Clinical Leader in the community as an essential manager, resident advocate, case manager, delegating and practicing RN.

Areas Of Responsibilities

Ensure that Victory Housing’s Mission Statement takes precedence over all decisions and actions.

Assume responsibilities of the Case Manager/Delegating Nurse as defined in COMAR 10.07.14 Regulations.

Manage all aspects of Pharmacy and Medication Management, including monthly medication card changes; ensure accuracy of monthly medication delivery from Order to MAR; train staff on new medications, parameters, orders and special conditions; update Emergency Sheet (electronic and hard copy).

Collaborate with the Assistant Director to recruit, hire, mentor, train and evaluate Medication Technicians; supervise medication administration in accordance with Nurse Practice Act and regulatory requirements.

Document physician orders into POS/MAR per VHI policy.

Perform daily/weekly MAR checks (EMR/Hard Copy) to ensure proper administration and documentation.

Maintain overall quality of the resident care program in compliance with regulations and high nursing practice standards.

Work with staff on issues as RN; proactively intervene on status changes.

Implement a comprehensive Health and Wellness Program with QA and outcome measures to monitor trends and anticipate issues.

Schedule weekly health checks for residents and review medications, fall risk, vital signs, nutritional status, skin assessments, and overall clinical, psychological and social status; record information in Wellness Assessment Forms and Progress Notes.

Maintain regular communication with residents, families, and physicians; serve as primary liaison with healthcare professionals when status changes occur.

Participate in monthly care planning, QA meetings, Family Clinical Reviews, and Hospice meetings; contribute to Monthly Staff Meetings and in-service trainings as requested.

Coordinate dietary and environmental needs with relevant staff; assist ED/AD with employee concerns and performance reviews.

Support clinical needs of residents 24/7 and oversee infection control, QA, and outcomes monitoring programs.

Review resident charts regularly for content, completeness and care appropriateness; ensure chart quality and regulatory compliance.

Instruct caregiving staff on clinical and nursing issues; assess prospective residents prior to move-in with ED.

On move-in day or within 24 hours, meet with the resident to discuss Wellness Check-ups, establish baseline, start clinical chart and schedule care needs in coordination with ED/AD/RSD.

Complete clinical components and follow-up of resident service plans with ED and caregiving staff; complete assessments required by COMAR regulations.

Coordinate services from outside agencies (home health, physical therapy, hospice, labs, DME, etc.).

Review and sign off on Incident Reports with ED and Assistant Director; document follow-up interventions.

Attend classes and meetings to stay current with nursing theory, trends and regulatory changes.

Scheduled hours are 10:00 – 6:30 or 9:00 – 5:30 based on community preference and must be adhered to.

In the absence of ED/AD, the Health/Wellness/Delegating Nurse is responsible for day-to-day operations.

Other duties as assigned.

Requirements Must be current on all vaccinations including COVID-19 and receive CDC-recommended booster doses.

Job Qualifications

Graduate of an accredited nursing program with three years of nursing experience, preferably in geriatric services.

Current Maryland RN license; baccalaureate degree in nursing or related health field with geriatric emphasis preferred.

Successfully completed the 20 Hour Delegating Nurse Course within 60 days of employment.
