Responsibilities
Screen, evaluate, and provide ongoing assessment of children eligible for early intervention services in collaboration with core team members.
Assist in developing an Individualized Family Service Plan (IFSP) for each child and deliver direct services addressing delays in physical, cognitive, communication, social, and adaptive development.
Instruct parents and caregivers by modeling strategies to integrate into family routines.
Problem solve and discuss progress and challenges with the team and prepare transition/discharge plans.
Qualifications
Graduate from accredited Physical Therapy program
NJ Licensed Physical Therapist
Maintain valid NJ driver's license and required insurance
At least 2 years' experience working with young children with disabilities
Targeted Evaluation (TET) team members require 2 years pediatric experience working and evaluating children ages 0-5
Sound background in normal child development
Knowledge of the educational and therapeutic value of an early intervention for children with identified developmental delays
Knowledge of the basic principles of behavior management
Ability to conceptualize, interpret and integrate early childhood educational and intervention techniques into family routines
Certifications and Licenses
NJ Licensed Physical Therapist
NJ Driver's License

40 hours/week
Essential Functions
Completes developmental evaluations in the child's natural environment and identifies strengths and needs.
Communicates effectively with families and team members in oral and written formats, both formally and informally.
Supports and enhances the family's ability to engage with their child during daily routines.
Implements measurable, functional outcomes as part of the IFSP.
Makes recommendations for ongoing services in collaboration with family, service coordination, and other team members.
Uses NJ Birth to Three Early Learning Standards as a framework for developmentally appropriate expectations.
Addresses IFSP outcomes by selecting and adapting activities and incorporating evidence-based intervention models.
Evaluates effectiveness of methods while meeting family outcomes.
Maintains confidentiality and adheres to NJEIS Code of Conduct.
Completes session notes and records data according to departmental rules; submits billing and required paperwork timely.
Maintains ongoing communication with office via email, text, and phone; responds promptly to information requests.
