Overview

The Center for Development and Disability (CDD) is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research and Service. The RN Case Manager (Senior) provides comprehensive case management services to medically fragile individuals and their families across New Mexico, primarily based in the Farmington, San Juan and Rio Arriba County area. The RN Case Manager assists with anticipatory guidance around care needs and transition planning, and helps navigate community-based health care services. Contacts are made through phone visits, telehealth visits, and home or community visits. The RN Case Manager collaborates with the individual, family and an interdisciplinary health care team to develop a comprehensive health care plan for evaluation, implementation and coordination of services from the health care team and community providers. Annual level of care assessments are completed and individualized interdisciplinary team meetings are conducted for each individual, with deadlines to ensure continuity of care. Responsibilities

Travel statewide to serve rural and frontier areas as needed; primarily based in the Farmington, San Juan and Rio Arriba County area. Provide case management services to medically fragile individuals and families, including anticipatory guidance and transition planning. Coordinate with the interdisciplinary health care team and community providers to develop and implement comprehensive health care plans. Complete annual level of care assessments and participate in individualized interdisciplinary team meetings for each individual. Maintain deadlines to ensure continuity of care and timely services. Minimum Qualifications

Bachelor’s degree in Nursing or a related clinical field; at least 1 year of directly related experience. State of New Mexico Licensed Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state with multistate privilege to practice in New Mexico. Completed degrees from an accredited institution above the minimum education requirement may be substituted for experience on a year-for-year basis. Preferred Qualifications

Experience with pediatric and/or community health nursing; home visiting; accessing resources and networking with the community and the interdisciplinary team. Experience working with children/families and/or individuals who are medically fragile or have special health care needs; leading and/or collaborating within an interdisciplinary team; partnering with community providers (physicians, PCPs, schools, hospitals, programs, therapists, nursing agencies, state agencies); and experience training other nurses and/or case managers. Bilingual Spanish/English. Preference for candidates living in or near Farmington, San Juan and Rio Arriba County to support local needs. Additional Requirements

Travel to different regions of New Mexico for face-to-face visits; based in Farmington, San Juan and Rio Arriba County area. Background check and fingerprinting required; passing of post-offer, pre-employment physical examination and medical history check. Conditions of Employment

Possession of a valid New Mexico driver’s license. CPR certification. May be required to submit to annual TB and rubella screening.
