The Center for Development and Disability (CDD) is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research and Service. These centers are authorized by the Developmental Disabilities Assistance and Bill of Rights Act to build the capacities of states and communities to respond to the needs of individuals with developmental disabilities and their families. This position travels to different regions of New Mexico and is primarily based in the Farmington, San Juan, and Rio Arriba County areas. The Medically Fragile Case Management Program (MFCMP) hires RN Case Managers to provide comprehensive case management services across New Mexico for medically fragile individuals and their families. The RN Case Managers support anticipatory guidance around care needs and transition planning and assist in navigating and accessing community-based health care services. Contacts are made through phone visits, telehealth visits, and home or community visits. The RN Case Manager works with the individual, family and the interdisciplinary health care team to develop a comprehensive health care plan and to coordinate services from the health care team and community-based providers. Annual level of care assessments and individualized interdisciplinary team meetings are completed for each individual and must meet deadlines to ensure continuity of care. RN Case Management services are provided statewide and require travel to rural and frontier areas as needed. Preference is for two or more years in pediatric and/or community health nursing, with experience related to medically fragile individuals and developmental disabilities. The RN Case Manager must be able to work autonomously while also functioning as a team leader and engaged interdisciplinary team member. This position travels to different regions of New Mexico, but is primarily based in the Farmington, San Juan and Rio Arriba County areas. The RN Case Manager collaborates with the individual, family and interdisciplinary team to plan and implement care and coordinate with community providers. Responsibilities

Provide comprehensive case management for medically fragile individuals and their families in coordination with MFCMP, the Medically Fragile Waiver, and MCOs. Develop and implement comprehensive health care plans with the individual, family, and interdisciplinary team; coordinate services from health care providers and community-based organizations. Perform annual level of care assessments and facilitate interdisciplinary team meetings; meet established deadlines to ensure continuity of care. Travel as needed to rural and frontier regions across New Mexico; conduct visits via phone, telehealth, and in person. Demonstrate autonomous work skills while leading and participating as a member of the interdisciplinary team. Minimum Qualifications

Bachelor's degree in Nursing or a related clinical field; at least 1 year of directly related experience. Must hold State of New Mexico Licensed Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the multistate privilege to practice compact with New Mexico. Degree substitutions on a year-for-year basis are allowed for completed degrees above the minimum education requirement. Preferred Qualifications

Experience with pediatric and/or community health nursing; home visiting; accessing resources and networking with the community and the IDT. Experience working with children/families and/or individuals who are medically fragile or have special health care needs. Experience as a leader and member of an interdisciplinary team (IDT) and in coordinating with community providers (physicians, PCPs, schools, hospitals, therapists, nursing agencies, state agencies). Experience with other nurses and/or case managers in training, daily work, and prioritization of duties. bilingual Spanish/English. Preference for candidates living in or near the Farmington, San Juan, and Rio Arriba County area to support local needs. Additional Requirements
