Overview
The Medically Fragile Case Management Program (MFCMP) hires RN Case Managers to provide comprehensive case management services across New Mexico for individuals who are medically fragile and their families. The RN Case Managers assist the individuals and families by providing anticipatory guidance around care needs and transition planning, as well as assisting in navigating and accessing community-based health care services. Contacts are made through phone visits, telehealth visits, and home or community visits. The RN Case Manager works with the individual, family and the interdisciplinary health care team to develop a comprehensive health care plan to assist in the evaluation, implementation and coordination of services from the health care team and community-based providers. RN Case Managers complete annual level of care assessments and individualized interdisciplinary team meetings for each individual and must meet certain deadlines to ensure continuity of care. Responsibilities
Provide comprehensive case management services across New Mexico for medically fragile individuals and families. Communicate with individuals and families via phone visits, telehealth visits, and home or community visits. Work with the individual, family and the interdisciplinary health care team to develop a comprehensive health care plan and coordinate services from health care team and community-based providers. Complete annual level of care assessments and participate in individualized interdisciplinary team meetings for each individual; meet deadlines to ensure continuity of care. Travel to rural and frontier areas as needed; position primarily based in the Albuquerque Metro area. Minimum Qualifications
Bachelor\'s degree in Nursing or a related clinical field; at least 1 year of experience directly related to the duties and responsibilities specified. Licensure: State of New Mexico Licensed Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the multistate privilege to practice compact with New Mexico. Completed degrees from an accredited institution above the minimum education requirement may be substituted for experience on a year-for-year basis. Preferred Qualifications

Experience with an interdisciplinary team (IDT) as a team leader and team member; experience with community providers (physicians, PCPs, schools, hospitals, FIT programs, therapists, nursing agencies, state agencies). Experience with other nurses and/or case managers in terms of training, daily work and prioritization of duties. Bilingual Spanish/English. Additional Requirements
