Overview
Presbyterian Homes & Services – Optage Hospice is looking for registered nurses who feel called to hospice and wish to serve older adults within a caring Christian environment. We are seeking a registered nurse to join our team as an RN Case Manager working in the East metro area. This is a full-time day position Monday through Friday, with equitable on-call rotation. The role focuses on care for older adults at the end of life—physically, emotionally, socially, mentally, and spiritually. $3,000 Hiring Bonus for FT Case Managers
Responsibilities
Ensure quality and safe delivery of hospice services to clients in their home, skilled nursing facility, or assisted living setting. Develop, implement, and evaluate the plan of care; make revisions to address problems. Coordinate hospice services and maintain continuity of care with other health professionals; identify interdisciplinary care needs. Utilize assessment, intervention, and teaching skills to maximize patient comfort and enhance the quality of life for patients and families. Provide ongoing assessments of physical, psychological, social, and spiritual needs; educate, supervise, and counsel clients and families as appropriate; document services accurately.
Qualifications
Registered nurse with current licensure in the state(s) of practice. A minimum of three years nursing experience preferred; background in critical care/oncology, long-term care, or medical/surgical with strong pain and symptom management knowledge preferred. Hospice/Home Care experience is a plus but not required. Excellent interpersonal and communication skills; ability to work with dying patients and caregivers in home, hospital, and other facility settings. Ability to work as part of an Interdisciplinary Team; strong assessment skills; computer skills helpful. Reliable transportation with valid driver’s license and auto insurance; demonstrated alignment with PHS’s mission and operating philosophies. Must be able to read, write, speak, and understand English to communicate with all customers.
Pay Range
The estimated starting pay range for this role is $76,398 - $95,514 per year. This is a good-faith estimate based on PHS’s typical starting pay range and may vary by qualifications and market conditions.
