Columbus House opened its doors in 1982 to provide services for adults experiencing homelessness. The organization serves men and women 18 years and older and focuses on helping individuals overcome homelessness through outreach, shelter, housing, and personal growth. Objective
Under the direction of the Manager of Emergency Service Case Management, the Family Triage Case Manager provides case management to homeless families (mothers and children). This role networks with the greater New Haven shelter system and mental health/substance abuse/social/medical/housing service systems and collaborates with clinicians and other Columbus House Case Managers to empower Family Triage clients to end their homelessness. Purpose
Under the direction of the Family Triage Coordinator, the Case Manager Aide (CM Aide) provides supervision, safety, organization, and program management support to families at the seasonal Family Triage Shelter. The CM Aide networks with the greater New Haven shelters and social service system and collaborates with the Case Manager and other Columbus House Residential staff to reflect the core values of Trauma Informed Service: Safety, Trustworthiness, Choice, Collaboration and empowerment. Qualifications, Knowledge, Skills And Abilities
• High School Graduate or equivalent; demonstrated ability to manage, motivate and direct consumers. • Knowledge of residential services for the homeless population and strong organizational and time management skills. • Proficiency with Microsoft Office and common desk top applications. • Ability to work with local networks of substance abuse, mental health, and social services providers. • Familiar with mental health, substance abuse and dual diagnosis issues and group dynamics; knowledge of local housing and vocational/entitlement programs. • Culturally competent; able to work within a team and foster teamwork. • General understanding of crisis intervention, confidentiality, boundaries, universal precautions; able to meet documentation requirements. • Able to foster a Harm Reduction philosophy; strong empathic skills, reliability, and punctuality. • Comfortable working with young children. Responsibilities
• Perform regular wellness rounds. Supervise up to 18 families to ensure compliance with all Triage rules. • Support and engage clients to assist them with following their person-centered plan. • Observe and document all shift communication logs. • Assist with moving client belongings as necessary. • Clean and disinfect surfaces and all open areas. • Collaborate with the coordinator to ensure living areas are prepared for new admissions. • Collaborate with the case manager, other agencies and social service providers. • Attend agency and program meetings as scheduled. • Complete required reports/incident logs/case notes. • Enforce house rules and maintain incident logs. • Proficient with CT HMIS and data entry as directed. • Support case manager with pre-screenings, intake/admission, case notes, discharges, and service plan execution. • Remain on-site to perform essential functions during emergency situations as designated by the CEO. • Other related duties as assigned.
