Family Triage Case Manager
for a temporary position from December 1, 2025, to TBD. The job is full-time seasonal. Company Overview & Mission : Columbus House provides life-saving outreach, shelter, and housing to people experiencing homelessness or at imminent risk, fostering personal growth and independence. The mission is to end homelessness in the community. Objective : The Family Triage Case Manager will provide case management to homeless families, network with the shelter system and service providers, and collaborate with clinicians to empower clients to end their homelessness. Qualifications : A Bachelor’s degree in human services is required, with at least two years of experience in human services or related fields. The candidate should have excellent organizational and time management skills, strong written and oral communication skills, and proficiency in Microsoft Office. Bachelor’s degree in human services required At least two years of experience in human services or related fields Excellent organizational and time management skills Strong written and oral communication skills Proficiency in Microsoft Office Able to effectively use the local network of substance abuse, mental health, and social services providers Familiar with criminal justice, mental health, substance abuse, and dual diagnosis issues Complete knowledge base of person-centered case management Able to meet all documentation requirements Knowledgeable about crisis intervention, boundaries, confidentiality, universal precautions, harm reduction, trauma-informed care, and motivational interviewing Valid driver’s license with a good driving history Responsibilities : The Family Triage Case Manager will provide linkages for clients who need social, mental health, educational, vocational, recreational, spiritual, and substance abuse services, provide transportation, coordinate with other agencies, oversee intake and daily roster, attend meetings, and maintain accurate documentation. Provide linkages for clients who need services Provide transportation to appointments Coordinate and collaborate with other agencies and social service providers Oversee intake, departures, and daily roster Attend daily Family morning call and weekly/monthly agency meetings Maintain accurate documentation and charting Enter and maintain client charts in the Homeless Management Information System Database Other related duties assigned Salary Range : $21.50-$22.50 per hour.
Schedule : Sunday-Thursday 2pm-10pm or Tuesday-Saturday 2pm-10pm.
