Summary The Advanced Practice Nurse Practitioner – Home Based Primary Care practices in the Medicine Service – Geriatrics and Extended Care to provide a full range of services emphasizing quality, timeliness, and the advancement of the VA mission. The NP provides advanced clinical practice, consultation, and management.

Duties

Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence‑based practice.

Travels in a government issued vehicle to meet with patients in their home environment (not in a clinic setting).

Develops and implements an individualized plan to achieve optimal population health and disease management.

Demonstrates leadership and works collaboratively with the interdisciplinary team.

Orders and interprets labs, tests, and diagnostic studies.

Prescribes pharmacologic and non‑pharmacologic interventions.

Initiates referrals and consults to specialty services.

Provides education to patients and their families.

Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.

Maintain a valid driver's license.

Licensure & Work Requirements

Full and unrestricted APN license in any U.S. state, territory, or the District of Columbia.

Active, current, full, and unrestricted state or territorial license to practice in Union of the United States, Commonwealth or District of Columbia.

Valid driver’s license; must travel in a government‑issued vehicle.

Full‑time: 8:00 am‑4:30 pm; Telework: not available.

Full‑time: 8:00 am‑4:30 pm; Virtual: this is not a virtual position.

Requirements

U.S. citizenship; non‑citizens may be appointed only if no qualified citizen can be recruited.

Urinalysis screening for illegal drug use prior to appointment; refusal results in denial of employment.

Proficient in basic written and spoken English.

Selective Service registration required for males born after 12/31/1959.

Background/security investigation required.

Completion of pre‑employment physical examination.

Participation in seasonal influenza vaccination program required.

Probationary period may be required.

Complete all application requirements including Identification forms (REAL‑ID compliant). Effective 7 May 2025 rules apply to driver’s licenses or state‑issued identity cards.

Must pass pre‑employment physical examination.

Qualifications

Education:

Post‑master’s, master’s, or doctoral degree as a Nurse Practitioner from an accredited professional nursing school (ACEN or CCNE). Those pending completion may be tentatively selected but not hired until requirements are met.

License:

All APNs must possess and maintain an active, current, full, and unrestricted APN license in any U.S. state, territory, the District of Columbia, or a Commonwealth.

Basic Board Certification:

Maintain full and current basic board certification in one of the following: Family NP (FNP), Adult NP (ANP), Adult‑Gerontology Primary Care NP (AGPCNP), or Gerontology NP (GNP) from ANCC, AANPCB, or another nationally recognized certifying body.

Experience:

NP clinical experience, VA experience, overall clinical experience considered preferred.

Physical Requirements Walking, bending, stooping, and lifting in direct patient care. Extended periods of sitting may occur.

Education

Transcript must be submitted with application if qualifications are based on education.

Only education or degrees recognized by the U.S. Department of Education from accredited institutions may be used to qualify for federal employment.
