St. Luke’s Health System in McCall, ID is seeking a Home Health Case Manager to join our Home Health team. The Case Manager will occasionally be required to provide support to Hospice patients in the McCall area. Position Overview
As a Home Health Case Manager, you will collaborate with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within the McCall and surrounding regions to deliver in-home care from admission to discharge. Full-time Schedule: Monday to Friday, 8:30 AM - 5:00 PM, with rotating evening on-call and weekend shifts Patient Population: Home Health and Hospice patients within the McCall service area Manageable Caseloads: Daily visit requirements are below the state and national average Compensation: Paid hourly which includes drive time and mileage reimbursement for traveling within McCall and its surrounding regions Additional Pay: On-call pay and applicable differential pay Bonuses: Sign-on bonus and relocation assistance available (where applicable) How We Support Your Development

Education: ASN or BSN in Nursing Experience: 2 years of RN experience; Home Health or Acute Care experience preferred Licenses/Certifications: Current RN license in the state of Idaho; Basic Life Support (BLS) certification Other Requirements: Valid driver’s license and proof of auto insurance Why St. Luke's
