OPPORTUNITY
Central Minnesota Mental Health Center (CMMHC) has a rewarding opportunity to serve as a
Mental Health Practitioner within our St Cloud ACT team.
This position provides a continuum of recovery-oriented, skill-building interventions under the supervision of a Mental Health Professional.
WHAT IS ASSERTIVE COMMUNITY TREATMENT?
Assertive Community Treatment (ACT) is an intensive, non-residential mental health service for persons with serious mental illness. These individuals often face barriers such as co-occurring substance abuse, homelessness, and unemployment. ACT teams help individuals treat and manage their mental illnesses and develop skills for community living.
ACT Services Are
Provided by a multidisciplinary team
A blend of treatment, rehabilitation, therapeutic, and supportive services
Directed to persons with a serious mental illness requiring intensive services
Offered on a time-unlimited basis, available 24/7, year-round

Duties And Responsibilities
Implement services according to the treatment plan under clinical supervision
Travel to client homes or community locations and transport clients as needed
Establish therapeutic relationships with clients
Provide medication monitoring and health education to promote healthy lifestyles
Complete assessments and maintain documentation
Coordinate with other programs and providers
Requirements
WHAT YOU’LL NEED
Qualifications as defined in Minnesota Statute 245.462, subdivision 17
Clear DHS background check and sexual exploitation screening

Experience with ACT teams is preferred
Summary
COMPENSATION
Mental Health Practitioner: $20.50 - $23.00
Mental Health Practitioner + Bachelor’s Degree: $22.50 - $25.00
Mental Health Practitioner + Master’s Degree: $24.50 - $27.00
(Salary varies with experience and degree in behavioral sciences or related fields)
SCHEDULE
Full-Time: Monday – Friday, 7:30 AM – 4:00 PM, with rotating on-call evenings and weekends
