Job Overview The Wellness Nurse is responsible for monitoring the health and well-being of residents and provides leadership support and assistance with resident care and services. Responsibilities include the overall well-being of residents, training and monitoring of medication care administration, communications with resident support services and families, and maintaining clinical quality assurance while leading Sunrise’s Mission to champion the quality of life for all seniors. Essential Duties

Coordination of Health Needs

Identify ongoing needs and services of residents to deliver high-quality resident services. Ensure proper follow-through, assessment, and documentation for changes in health care services and status. Assist Resident Care Director (RCD) in conducting resident assessments at move-in, 30 days post-move in, semi-annually, and with any status change. Conduct monthly wellness visits for all residents. Assess and follow up with residents after falls, accidents, or incidents; document and report as required. Document pertinent information in the resident wellness file. Maintain communications with residents’ families to ensure medical needs are met. Contact residents’ attending physicians as necessary or upon family request. Ensure weights and vital signs are obtained monthly for each resident. Maintain wellness files per Sunrise policies and regulatory requirements. Maintain medical supplies and emergency kits for the community. Provide clinical support and assistance to team members as needed. Understand and follow infection control practices. Promote infection control standards with the RCD and team, including resident testing, training, and skills checks. Practice proper handling and disposal of blood borne pathogens, bodily fluids, and hazardous materials in compliance with universal precautions.

Medication Programs

Monitor each resident’s medication and treatment profile to ensure orders are followed and documented accurately. Transcribe physician orders into MAR/TAR with initials, date/time, and pharmacy communication. Complete MAR/TAR audits monthly. Demonstrate knowledge of the Six Rights of Medication Pass. Serve as liaison for pharmacy services to ensure effective resident services. Monitor the Medication Room and carts for order, cleanliness, and expired medications. Complete medication observations and skills checks for medication care managers as directed by the RCD.

Quality Assurance and Regulatory Compliance

Demonstrate knowledge of quality improvement areas: Resident Centered Care Model, APIE, and Quality Care Indicators and Outcomes; participate in Sunrise Quality Assurance and Clinical Meetings. Demonstrate knowledge of regulations: federal, state/provincial, local resident care standards; Resident Rights; and Resident Assessment/Evaluation processes. Review physician reports of new residents as directed by the RCD; order medications and equipment as directed. Adhere to Sunrise practices and guidelines in resident care programs (skin, nutrition, behavior, falls, bowel/bladder). Assist in implementation, training, and monitoring of documentation related to residents’ clinical health and wellness. Report resident changes, concerns, and solutions to the RCD as appropriate. Contribute to engagement programs and processes, including team member surveys and improvement planning. Attend regular meetings (Town Hall, Department Team, Cross Over, etc.) as directed. Maintain compliance in required training and regulatory authorities’ requirements; perform other duties as assigned.

Experience and Qualifications

To perform this job successfully, the individual must perform each essential duty satisfactorily. The requirements listed are representative of the knowledge, skills, and abilities required. Graduate of an approved college/school of nursing Current state/provincial Registered Nurse (RN) license Minimum of one (1) year of experience in home health, assisted living, hospital, or long-term care Knowledge of nursing practices, techniques, and methods for health and wellness resident care; understanding of federal, state/provincial, and local regulations Strong assessment skills Proficiency in computer skills, Microsoft Office, and Sunrise applications; ability to learn new applications Ability to work weekends, evenings, and flexible hours to meet service delivery needs About Sunrise
