Sunrise of San Rafael . The Wellness Nurse is responsible for monitoring the health and well-being of our residents and provides leadership support and assistance with resident care and services. The role supports the Mission of Sunrise Senior Living: to champion the quality of life for all seniors, in accordance with federal, state/provincial, and local standards and Sunrise policies. Job ID: 2025-229246 Responsibilities

Coordination Of Health Needs Identify ongoing needs and services of residents to promote high-quality resident services. Ensure proper follow-through, assessment, and documentation for changes in health care services and status. Assist Resident Care Director (RCD) in conducting resident assessments at move-in, 30 days post move-in, semi-annually, and with any change in status. Conduct monthly wellness visits for all residents. Assess, monitor, and follow up on falls, accidents, or incidents; document and report as required. Document pertinent information in the resident wellness file and maintain communications with families or responsible parties. Contact attending physicians when necessary or upon family request. Ensure weights and vital signs are obtained monthly for each resident. Maintain wellness files according to Sunrise policies and applicable regulations. Maintain medical supplies and emergency kits for the community. Provide clinical support and assistance to the community team as needed. Understand and practice infection control; promote infection control standards and resident testing where applicable. Understand proper methods for handling bloodborne pathogens, bodily fluids, and hazardous materials in compliance with universal precautions. Medication Programs Monitor each resident's medication and treatment profile to ensure accuracy and as ordered. Transcribe physician orders to MAR/TAR and ensure timely documentation and pharmacy communication. Complete MAR/TAR audits monthly. Demonstrate knowledge of the Six Rights of Medication Pass (Right resident, medication, dosage, time, route, right to refuse). Act as liaison for pharmacy services; monitor medication rooms and carts for accuracy and safety. Complete medication observations and skills checks as directed by the RCD. Quality Assurance and Regulatory Compliance Demonstrate knowledge in: Resident Centered Care Model, APIE, Quality Care Indicators and Outcomes; participate in Sunrise Quality Assurance and Clinical Meetings. Ensure compliance with all Federal, State/Provincial, and Local regulations; respect Resident Rights and the Evaluation process per regulatory requirements. Review physician reports of new residents as directed by the RCD; order medications and equipment as directed. Support documentation related to resident health and wellness; report changes or concerns to the RCD as appropriate. Contribute to engagement programs and attendance at meetings as directed. Maintain required training compliance and Sunrise standards. Core Competencies Ability to handle multiple priorities; strong written and verbal communication; ability to teach and guide team members. Organizational and time-management skills; good judgment, problem solving, and decision-making skills. Experience And Qualifications Graduate of an approved college/school of nursing; current state/provincial RN license. Minimum of one year of experience in home health, assisted living, hospital, or long-term care. Knowledge of nursing practices, regulations, and assessment skills; proficiency with computers and Sunrise applications. Ability to work weekends, evenings, and flexible hours as needed.
