Wellness Nurse RN role at Sunrise of San Rafael. This position supports resident health and well-being, provides leadership in resident care, and aligns with Sunrise Senior Living’s mission to champion the quality of life for all seniors. Responsibilities
Coordination Of Health Needs
Identify ongoing needs and services of residents to promote the highest quality resident services. Ensure proper follow through, assessments, and documentation for residents with changes in health care services and status. Assist Resident Care Director (RCD) in conducting resident assessments at move-in, at 30 days post move-in, semi-annually, and with any status change. Conduct monthly wellness visits for all residents. Assess, monitor, follow up, and document for residents who have experienced a fall, accident, or incident; report as required. Document all pertinent information in the resident wellness file. Maintain communications with residents’ families or responsible parties to ensure medical needs are met. Contact the resident’s attending physician when necessary or upon family request. Ensure weights and vital signs are obtained monthly for each resident. Maintain wellness files according to Sunrise policies and federal and state/provincial regulations. Maintain medical supplies and emergency kits for the community. Provide clinical support and assistance to community team members as needed. Understand and follow infection control practices and promote infection control standards with the RCD and team, including resident testing, training, and skills checks. Practice proper methods for attending to and disposing of, and the handling of exposure to bloodborne pathogens, bodily fluids, infectious waste, and hazardous materials in compliance with universal precautions. Medication Programs
Monitor each resident’s medication and treatment profile regularly to ensure administration as ordered and documentation accuracy. Transcribe physician orders to MAR/TAR with initials, date, time, and verify accuracy for pharmacy. Complete MAR/TAR audits each month. Demonstrate knowledge of the Six (6) Rights of Medication Pass: Right resident, right medication, right dosage, right time, right route, and right to refuse. Act as liaison for pharmacy services to ensure effective services for residents. Monitor the Medication Room, medication carts, and treatment carts for neatness and accuracy, and manage expired medications. Complete Medication Observations and skills checks for medication care managers as directed by the RCD. Quality Assurance and Regulatory Compliance
Demonstrate knowledge in key quality improvement areas: Resident Centered Care Model, APIE, Quality Care Indicators and Outcomes, and Sunrise Quality Assurance/Clinical Meetings. Demonstrate knowledge of regulations: All Federal, State/Provincial, and Local resident care regulations; Resident Rights; Resident Assessment/Evaluation processes per regulatory requirements. Review Physician Reports for new residents as directed by the RCD and order medications and equipment as directed. Demonstrate knowledge of Sunrise practices and guidelines for resident care programs (skin, nutrition, behavior, falls, bowel/bladder). Assist in implementation, training, and monitoring of documentation related to residents’ clinical health and wellness; report changes to the RCD as appropriate. Contribute to engagement programs and processes, including team member engagement surveys and improvement planning. Attend regular meetings as directed and maintain required training to meet Sunrise standards. Perform other duties as assigned. Core Competencies

Graduate of an approved college/school of nursing Current state/provincial RN license Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care Knowledge of nursing practices, guidelines, and applicable regulations Knowledge of good assessment skills Proficiency in computer skills (Microsoft Office, Sunrise applications) and ability to learn new applications Ability to work weekends, evenings, and flexible hours as needed for peak service delivery About Sunrise
