Job Summary & Qualifications

Provides case management services for inpatient and observation patients as assigned.

Identifies patients at risk for adverse outcomes during care transitions.

Performs comprehensive psychosocial, medical, and discharge needs assessment of patients/family.

Reassesses the patient’s clinical condition, considering readmission risk and developing mitigation strategies.

Coordinates the plan of care and drives the discharge plan in collaboration with physicians and the multidisciplinary team.

Partners with Social Services to ensure appropriate post‑acute medical needs and level of care.

Assumes responsibility for timely referral to Social Services when psychosocial determinants of health are identified.

Involves patient, family, and significant others in clarifying needs and expectations to develop realistic goals.

Evaluates progression of care using evidence‑based tools and approved criteria (InterQual) throughout the episode of care and escalates transition of care issues through the chain of command.

Makes appropriate referrals to third‑party payers and disease and case management programs for recurring patients and patients with chronic disease states.

Facilitates patient throughput with an ongoing focus on effective care transitions, quality, and efficiency.

Documents professional recommendations, discharge plan, care coordination interventions, and case management activities for effective communication to team members.

Aligns patient needs with available resources to ensure a safe discharge/transition.

Acts as a liaison through professional communications between physicians, patients/family, hospital staff, and outside agencies.

Actively seeks ways to control costs without compromising patient safety, quality of care, or services delivered.

Directs activities to identify and provide for the needs of the under‑resourced patient population, including patient education, assistance programs, and community‑based resources.

Participates in performance improvement activities, documenting and intervening when avoidable days occur.

Adheres to established policy and procedure and standards of care, escalating issues promptly through the chain of command.

Demonstrates knowledge of regulatory requirements, HCA ethics, and compliance policies, and quality initiatives.

Serves as an advocate for patient rights, needs, and values, ensuring consideration of ethnic, cultural, or religious values and preferences.

Performs other duties as assigned and practices the Code of Conduct.

Qualifications You Will Need

Associate Degree in Nursing or Nursing Diploma (Required).

Bachelor’s Degree in Nursing (Preferred).

2+ years experience in case management OR 3+ years experience in clinical nursing (Required).

InterQual experience (Preferred).

Location HCA Florida Twin Cities Hospital is a 65‑bed facility located at 85 North and College Boulevard in Niceville. The hospital offers private rooms, the only local certified orthopedic joint replacement program, and certified spine surgery program.
