Duties
Independently manages a panel of patients with acute and chronic health conditions, with an emphasis on health promotions and disease prevention using evidence‑based practices.
Develops and implements individualized plans to achieve optimal population health and disease management.
Demonstrates leadership and works collaboratively with the Primary Care Team and other interdisciplinary team members.
Orders and interprets labs, tests, and diagnostic studies.
Prescribes pharmacologic and non‑pharmacologic interventions.
Initiates referrals and consults to specialty services.
Provides education to patients and their families.
Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.

Physical examination required; must pass pre‑employment physical exam.
Seasonal influenza vaccination program participation required for all personnel.

Qualifications
Education:
A post‑master’s certificate, master’s, or doctoral degree as a Nurse Practitioner from an accredited nursing program.
License:
Active, current, full, and unrestricted Nurse Practitioner license in any U.S. state, territory, or Commonwealth.
Board Certification:
Full and current basic board certification in one of the recognized categories (FNP, ANP, AGPCNP, ACNP, AGACNP, GNP).
Scope, Education, and Experience:
Meets the basic education requirement and, for Advanced Practice Nurse II and III, follows the specified dimension requirements such as clinical practice, outcome focus, leadership, professional development, and professionalism.
Education MSN required.
