Case Manager RN – WelbeHealth
WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community‑based services. The Case Manager, RN directs participant care as a crucial voice on the interdisciplinary team (IDT). Reporting to clinical leadership, the RN delivers and oversees all aspects of care, including required PACE assessments, personalized care plans, and care coordination transitions. Conduct nursing assessments to determine participant needs, preferences, and goals Participate in IDT meetings to develop and revise participant care plans Deliver and document nursing interventions, maintain a safe environment, and promptly respond to physician orders Collaborate with the PACE Physician and IDT to coordinate participant care needs and communicate with participants and families Instruct participants, families, and caregivers on disease processes, self‑care techniques, and prevention strategies Qualifications
Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred Unencumbered California RN license Current Basic Life Support (BLS)/First Aid certification required Two (2) years of nursing experience in a clinical setting with a frail or elderly population preferred Benefits & Compensation
Salary base range: $52.52 – $69.32 hourly + bonus + equity. WelbeHealth offers a competitive total rewards package including 401(k) match, comprehensive medical/dental/vision coverage, 17 days PTO, 12 holidays, 6 sick days, and additional benefits. COVID‑19 Vaccination Policy
