Breaking Ground operates nearly 4,000 units of housing across New York City, as well as in upstate New York and Connecticut. Our supportive housing combines affordable housing with services designed to help people maintain their homes long-term. This approach is recognized as an effective solution to chronic homelessness.
Beyond providing housing, our programs and services assist street homelessness, especially for those who have been homeless the longest. Through our Street to Home outreach and transitional housing resources, we help individuals secure and sustain permanent housing.
The Community Health Specialist, reporting to the Assistant Director of Programs, provides on-site and field assessments for the Safe Options Support (SOS) outreach teams in Staten Island. This role involves engaging with individuals to assess homelessness history, identify needed services, and determine suitable placement options. The specialist ensures the use of Harm Reduction and Housing First strategies, along with other best practices, to deliver high-quality care. The role includes managing a caseload and maintaining client records in compliance with regulatory and agency standards. The position primarily operates during business hours (Monday-Friday), with availability for off-hours emergencies or special projects. ESSENTIAL DUTIES AND RESPONSIBILITIES:
Manage a caseload of clients Assist with outreach and engagement of individuals experiencing homelessness and living with chronic medical conditions in Staten Island Implement case management strategies based on Housing First, Harm Reduction, Critical Time Intervention (CTI), Motivational Interviewing, and trauma-informed care Develop and update individual service plans; document client contacts and progress thoroughly and promptly Address challenges in client engagement and behaviors through troubleshooting and problem-solving Coordinate care with multiple service providers, including outreach teams and housing providers, and conduct case conferences as needed Perform additional duties as assigned MINIMUM QUALIFICATIONS:
NYS Licensed Master Social Worker (LMSW) or NYS Licensed Mental Health Counselor (LMHC) strongly preferred. Minimum requirements: MSW, MHC, MPH, or MPA Experience with related populations highly preferred Strong clinical understanding of service delivery models including Housing First, Motivational Interviewing, Harm Reduction, and behavioral/medical health systems, with the ability to navigate and access services for clients Experience in crisis intervention, risk assessment, and incident management Ability to collaborate effectively with diverse stakeholders Proven success working in fast-paced environments with multiple priorities Excellent verbal and written communication skills with diverse populations and stakeholders Valid driver’s license for NY, NJ, CT, or PA with a good driving record, as defined by Breaking Ground Proficiency in Microsoft Office Suite (Word, Excel, Outlook, Teams); familiarity with Access databases and quick learner for new software
