In this role, as the QRT Navigator at Hamilton County, you will provide direct client service and case management for individuals and families impacted by substance use disorders. You will connect clients with appropriate treatment, mental and physical health services, housing resources, and support systems while maintaining accurate records and collaborating with community partners to ensure seamless service delivery.
Summary of Job Duties
Client Support and Services Navigation: Implement strategies to guide clients through systems of care, including prevention, triage, service linkage, follow-up, and family support.
Assessment and Case Management: Evaluate and prioritize client needs, connecting them with appropriate treatment and resources to support long-term stability.
Record Keeping and Data Management: Maintain accurate and up-to-date client records using the RMS System (Cordata) and the RedCap Harm Reduction Tracking System.
Community Collaboration and Resource Coordination: Build and maintain relationships with HC ARC partners and community organizations to enhance access to services.
Budget Management and Administrative Duties: Monitor expenses (parking and mileage, costs incurred during work trips, bus pass account, Uber account, etc.).
Team Coordination and Supervision Support: Proactively use the shared calendar to document activities; participate in daily briefings and debriefs with the co-responder team.
Program Development and Barrier Reduction: Identify gaps and barriers in client services and community resources using data-driven practices.
Minimum Qualifications
Bachelor’s degree in social work, sociology, psychology, or a related field.
One or more years of experience in social services, case management, or a related capacity.
Or equivalent combination of training and experience demonstrating knowledge of social services and case management practices.

40 hour work week, generally between core hours of 7:30 a.m. to 5:30 p.m.
