Overview Join an established Primary Care practice where you will perform patient assessments, establish care plans, and provide education. Clinic hours are Monday - Friday, 8am-5pm, with excellent work/life balance, no call, and no weekends.
Responsibilities
Perform patient assessments and assign diagnoses based on recorded data and current research.
Establish care plans and implement interventions according to protocols and standards.
Provide education and counseling to patients, families, and groups.

Qualifications
Master’s degree or higher in nursing or physician assistant studies
Current Illinois license as an NP or PA
Certification by ANCC or NCCPA
Minimum two years of primary care experience preferred
Current BLS and ACLS certifications
Proficiency in EMR systems
