HIM Patient Abstractor II – Health Information – FT Days
Location: UC Irvine, Orange County, CA (Day shift)
Overview: The incumbent is responsible for analyzing patient record content in order to accurately assign documentation deficiencies to the responsible physician; communicating and coordinating with medical and house staff regarding healthcare record deficiencies; and providing assistance to physicians in completing incomplete healthcare records and documents according to hospital by-laws, rules, regulations and JCAHO guidelines. Prepares and complies daily, weekly and monthly statistical reports, ensuring integrity of chart deficiency database and participates in departmental PI projects.
Responsibilities
Analyze patient record content and assign documentation deficiencies accurately.
Communicate and coordinate with medical and house staff regarding healthcare record deficiencies.
Assist physicians in completing incomplete healthcare records and documents per hospital by-laws, rules, regulations and JCAHO guidelines.
Prepare and comply with daily, weekly and monthly statistical reports.
Maintain integrity of chart deficiency database and participate in departmental PI projects.
Required Qualifications
Familiarity with alpha/numeric filing systems.
Ability to establish and maintain effective working relationships in the department and across the Health System.
Three years experience working with hospital medical records.
Preferred Qualifications
Working knowledge of EPIC registration system.
Working knowledge of BISCOM, RHIA or RHIT credential.
PC literacy.
Knowledge of medical terminology.
