About Partnership Health Center Partnership Health Center (PHC) is currently in the process of separating from Missoula County, with completion expected by the end of this year. As a result, our employee benefits package will be changing as early as January 1, 2026. PHC is a 14-site, Federally Qualified Health Center with Missoula County. Responsibilities
Provide essential medicine care to vulnerable, primarily unhoused patients in the Missoula Area.
Provide primary care to patients as an independent practitioner, diagnosing, treating, and managing acute and chronic health conditions.
Work closely with community partners to ensure coordinated care and support for patients.
Engage in outreach to identify and connect with unhoused individuals who may benefit from medical care; build trust and rapport to facilitate ongoing care.
Identify and address barriers to healthcare access; advocate for services and resources; perform medical history, physical and psychosocial examinations.
Formulate diagnoses and prescribe treatment, including medications.
Perform minor clinical procedures and review lab results with follow-up as necessary.
Maintain accurate patient records using SOAP format; handle billing and coding for billable visits.
Participate in quality assurance reviews and chart audits with the medical director.
Maintain expertise and certification, including CME hours.
Educate PHC team and partners on best practices for delivering healthcare in non-traditional settings.
Other Duties and Working Conditions
Performs related work as required; provide coverage at satellite locations as needed.
Education: Requires a current APRN or PA license to practice in Montana, or eligibility and willingness to relocate.
Experience: Requires two years in a clinical setting (health center preferred); one year experience with vulnerable/unhoused populations; ability to work in a multidisciplinary team.
Special Requirements: Immunizations or proof of immunity and TB test; vaccination status may be requested.
Supervision: Works under the PHC Medical Director, CMO, and designated physician mentor.
Supervision Exerted: No direct supervision; may supervise students during rotations.
Working Relationships: Frequent contact with the public and PHC staff; acts as an expert in health to inform and influence patient adherence.
Required Knowledge, Skills, and Abilities
Knowledge : In-depth knowledge of primary health care, pharmacology, public health concepts, HIPAA, and PHC Compliance Guidelines.
Skills : Proficiency in basic lab procedures, interpreting results, clinical examinations, charting, and related equipment.
Abilities : Sensitivity to diverse populations; ability to work independently; strong English communication; ability to work in mobile/non-traditional settings; physical capability for required exertion and exposure to biomedical hazards.
