Case Manager RN - Tallahassee, FL
Our client, a nationally recognized healthcare system, is seeking a
Case Manager RN
to join a dynamic care management team in an acute care setting. This role focuses on ensuring appropriate clinical care, resource utilization, and discharge planning while acting as a patient advocate. You’ll coordinate with physicians, staff, patients, and families to support high-quality outcomes and smooth care transitions across the continuum.

Focus on appropriate clinical care, resource utilization, and discharge planning as part of care management in an acute care setting.
Qualifications
3+ years recent acute care Case Management experience
(within last year preferred) or 3+ years’ experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED; OR home health/insurance case management with at least 3 years total acute care experience (1 year within last 5 years).
Associate Degree or Diploma in Nursing
(required);
BSN preferred
Current
RN license in Florida
or compact state (must obtain FL within 90 days if compact).
Advanced Practice RN license accepted if compliant.
Certification in Case Management, Nursing, or Utilization Review
preferred.
Strong communication, leadership, and care coordination skills.
Schedule & Benefits
Evening shift : 12:00 PM – 8:30 PM, with 1 weekend per 4-week schedule.
Evening and weekend differentials stackable ( $2.50 evenings ,
$2.00 weekends
– stackable).
$10,000 sign-on bonus
(paid in installments over 2 years).
