Summary of Duties Responsible for reviewing physician medication orders (EPIC CPOE) for appropriateness cost-effectiveness, monitor and adjust patient drug therapies according to many pharmacy driven protocols, and provide drug information to patients and other healthcare providers. Providing pharmaceutical care including anticoagulant, PK, renal dosing, IV to PO, insulin dosing, antimicrobial stewardship, and many other pharmacy protocols. Drug therapy management via established policies and procedures, ensuring that the pharmacy provides optimal services on a daily basis. Pharmacists will also be involved in quality improvement activities, distributive activities and overseeing techs, precepting PharmD students and PGY1 residents. In addition, may also be assigned other duties as required. Critical Care, Emergency Medicine, Cardiology interest & experiences are a plus.
Education Requirements Graduate of an ACPE-accredited School of Pharmacy, Doctorate in Pharmacy (PharmD) and completion of an ASHP accredited residency program or at least two (2) years of an inpatient clinical experience desired. ACLS certification desired. Strong knowledge of computer systems/EPIC, Pyxis, barcoding and inpatient workflow.
Licensure/Certification Requirements Current CAPharmacy license required. ASHP accredited Residency, ACLS, BLS preferred.
Experience Requirements Antimicrobial stewardship and transition of care experience preferred. Required at least two (2) years of prior recent inpatient clinical experience or completion of ASHP accredited residency required. Must be able to cover day, evening, or night shifts as needed, including weekends and holidays, and coverage for sick calls and vacations.
Special Skills/Abilities Effective communication skills (written and verbal) in English required. Must be proficient with Microsoft Office applications and proficient with EPIC desired. Good planning and organizational skills, as well as the emotional stamina to work in a stressful environment desired. Must possess computer and barcode knowledge, including pharmacy operations. Must be able to cover all shifts as needed.
