As a
Counselor , you will oversee all aspects of a patient’s treatment. You will be responsible for the coordination and continuity of care from admission through discharge and follow‑up, promote performance improvement, and deliver comprehensive psychological therapy to address the causes and effects of addiction through substance‑abuse counseling, family and group therapy, and related services.

Qualifications
Bachelor’s or master’s degree in psychology, counseling, or social work from an accredited college or university as required by the state
Three (3) years of experience in substance use, mental health, or a related field
Licensure or certification for counseling as required by the state (e.g., 12VAC30—130‑5020)
LCSW (Licensed Clinical Social Worker)
LPC (Licensed Professional Counselor)
LMFT (Licensed Marriage Family Therapist)

Responsibilities
Interface with the clinical team in decision‑making involving patients, attending all clinical meetings
Assess the counseling needs of the patient
Use DSM criteria to assess substance abuse diagnosis and determine appropriate level of care based on ASAM criteria
Obtain relevant previous records for the current treatment episode
Prepare the substance‑abuse counseling portion of the patient treatment plan
Provide substance‑abuse counseling services as specified in the treatment plan
Develop the patient discharge/continuum of care plan to ensure movement to appropriate levels of care
Participate fully as a member of a multidisciplinary team for assigned patients
Document all counseling and education services, assessments, reassessments, referrals, and follow‑up in the patient’s clinical record
Maintain patient caseload, including all case management and utilization review updates, treatment planning duties, and clinical charting as required
Coordinate support services for the patient and their family, recording the services in the clinical record
Reassess patients and revise the substance‑use counseling portion of the treatment plan as necessary
Facilitate family therapy as assigned
Coordinate with referral sources (e.g., legal, school, employer)
Facilitate intervention for crisis‑oriented issues using a multi‑disciplinary team approach
Carry out administrative and clinical functions as requested
Attend team meetings and complete all required training courses promptly
