Associate Marriage and Family Therapist
Location : San Leandro, CA
Schedule : Monday – Friday, 8 : 00 a.m. – 5 : 00 p.m.
Salary Range : $66,000 – $80,000
Position Summary We’re looking for an Associate Marriage and Family Therapist (AMFT) to support our Integrated Behavioral Health Program in San Leandro, CA. This role provides an opportunity to gain real-world experience in an interdisciplinary setting that bridges mental health and primary care. You’ll receive direct clinical supervision while helping families access the care they deserve.
Key Responsibilities
Deliver in-person and telehealth-based care coordination for individuals with behavioral health and social support needs
Conduct comprehensive client assessments and contribute to individualized care planning
Facilitate warm handoffs and referrals to behavioral health, medical, housing, and community-based resources
Monitor engagement and support follow-up care to maintain service continuity
Collaborate with internal care teams and external partners to address client needs holistically
Participate in multidisciplinary team meetings, case reviews, and service planning sessions
Document services and progress accurately in the electronic health record (EHR)
Assist with data collection and service tracking to meet reporting requirements
Compliance & Training
Ensure compliance with Medi-Cal, HIPAA, and internal documentation protocols
Attend required trainings, community forums, and IBH-related meetings
Support program evaluation activities by monitoring outcomes and service delivery
Contribute to outreach, resource navigation, and sustainability efforts
Requirements
Registered Associate Clinical Social Worker (ASW / ACSW) in the State of California
Bachelor’s or Master’s degree in Social Work, Psychology, Public Health, or a related field
Minimum 1 year of relevant experience in behavioral health, case management, or social services
Familiarity with Alameda County systems of care, including Medi-Cal and community-based resources
Understanding of trauma-informed and culturally responsive service delivery
Strong communication, time management, and interpersonal skills
Ability to work collaboratively with both clinical and non-clinical team members
Preferred
Experience in integrated or primary care settings
Proficiency in an electronic health record system (SmartCare or similar)
Bilingual skills (Spanish or other languages) highly valued
Reliable transportation and valid CA driver’s license
Work Environment
Hybrid community-based and clinic-based setting
Occasional travel within Oakland CA
Use of standard office and telehealth equipment
May require occasional evening or weekend flexibility
Supervision & Support
Clinical supervision available to support licensure hours
Ongoing training and mentorship provided through internal and external partners
