Case Manager, Maternal Home Visiting Program

Coming October 2025 (Pending Funding): Case Manager - Maternal Home Visiting Program. Washington, DC | Hybrid / On-site. Every mother deserves to feel supported, every child deserves a healthy start, and every family deserves someone in their corner. For many parents in DC, the journey through pregnancy and early childhood comes with challenges—navigating healthcare, securing stable housing, or managing the stress of raising a newborn without enough support. As a Case Manager with the Maternal Home Visiting Program, you’ll step directly into homes and lives at their most vulnerable and hopeful moments. You’ll provide trusted guidance, coaching, and advocacy—whether it’s teaching safe sleep practices, screening for postpartum depression, or helping a family connect to healthcare, WIC, or childcare resources. What You’ll Do

Provide regular home visits with education and coaching on prenatal care, infant health, breastfeeding, parenting, and safe sleep. Conduct assessments (health, psychosocial, developmental, safety) and create individualized care plans. Offer counseling and support for perinatal mood disorders and stress management. Connect families with healthcare, housing, education, employment, and social services. Maintain accurate case notes and track outcomes (prenatal care, immunizations, screenings). Advocate for families in schools, clinics, and community agencies. Participate in reflective supervision, peer case review, and professional development. Qualifications

Master’s degree required in Social Work, Counseling, Psychology, Maternal/Child Health, Nursing, or related field. At least 2 years of case management or counseling with pregnant women, infants, or young children. Knowledge of maternal/child health, trauma-informed care, and infant development. Strong communication, crisis intervention, and organizational skills. Ability to conduct home visits and work flexible hours. Preferred: DC licensure (LICSW, LPC, LMFT, RN), bilingual skills, and experience with evidence-based models (NFP, PAT, HFA). Expected Start Date: October 2025 (pending funding).
